After adding images, try to regenerate the thumbnails (in the gallery settings)
and don't forget to clear the cache

There can be no conflict, when loading for hosting, we check the destination folder and if there is a file with the same name, we add a set to the file file and check again if there is such a file in the folder, etc., until it becomes unique
I think this is most likely the problem is related to cache

When you see a problem, check the url of the image (open it in a new tab) and check which image is in the folder at the specified url

but you are adding images from joomla media manager, not from your computer.
the problem comes from when I upload pictures from my computer.
