Sometimes files aren't coming through

I've got a form for accepting file submissions, and the file selector is a required field. Sometimes, I get the email that someone submitted the form, and rather than including the file, it just shows [Field=Upload File 1] in the place of the title of the field. Any ideas.

By some reason file wasn't uploaded, as an example file was broken or can be different reasons.

I can recommend you - try to upload different files, maybe you'll find concrete reason
or you can send us Joomla admin access and we can test.

The form is at wisdomhomeschooling.com/forms/upload-files

If you look in the logs, at least a quarter of the submissions don't have a file attached (and the first file upload field is a required field).

From the client:
"I'd say I receive around 2/3 of the things people submit, in general. In the case below, she did two submissions. Each one was a notification form and a birth certificate. In both emails, the birth certificate came through, but NOT the notification form. She resubmitted notification forms in another attempt the next day, but neither one came through. "

Hi,

The client is reporting that some files aren't coming through again, and it's just showing "[Field=Upload File 1]" instead of the title of the field.

I've just submitted the login info.

The client said this:
The biggest problem is when someone attempts to send something but they don't know it hasn't sent.... and I haven't received anything at all so I don't know they're trying.

When I get blank submissions I email the sender to ask them to try again. It seems that pdfs go through easier than jpgs. And big jpgs don't seem to send even though the settings indicate that they should.

upload_max_filesize, post_max_size
