Balbooa Help Center: Form result shown after submitting (1/1)

Dear Support Team,

from a few days ago our form started to work not properly.
We have a form where users should fill an online test as you can see here: https://www.blc.co.hu/online-szintfelmero-teszt.
At first, I could fill it but after that there was no other chance to fill it again, because the form has shown the results but not the opportunity to fill it again.
I tried to fill it in diffenrent browsers but I always got back the results but not the radio buttons.

Could you help me please it would be very important.
