Page Title / URL in Email

Hi there,

Is it possible for the email received from the form to include the Page Title and/or the URL of the page the form was submitted from?

We would like to create one form, then add it to multiple pages, but need to know which page the user was on when submitting the form. The pages (events directory) will be changing frequently so creating a form for each page isn't really an option.
