ReCaptcha only shows on one of 4 forms

I have 4 pop up forms at www.tncodc.com/strategic-initiative. I have ReCaptcha enabled for all 4 (as required by the client) but it only shows up for the first pop up form at the top and not the other three. Also when submitting on those three I get a "message could not be sent" response which I assume is related to the fact that Captcha is enabled but not showing to be able to click it.

Also I have redirect urls set for the top three and the only one I can test is the first one that has the captcha and the redirecting is not working.

I have the notification email set to This email address is being protected from spambots. You need JavaScript enabled to view it. for now for testing if you want to try it out.
